Testosterone, Weight Loss, and Body Composition: What Men Need to Know

You're working out. You're watching what you eat. But the fat around your midsection won't budge, and the muscle gains you used to see from the gym have stalled or disappeared. You feel like you're putting in the same effort for a fraction of the results.

This is one of the most common experiences men bring to JS wellness — and in a significant number of cases, testosterone is a major part of the explanation.

How Testosterone Affects Body Composition

Testosterone is not just a hormone of sexual function and masculinity. It's a key metabolic regulator that directly governs how your body handles fat and muscle:

•       Anabolic effects — testosterone stimulates muscle protein synthesis, meaning it drives the building and maintenance of lean muscle mass

•       Fat metabolism — testosterone supports fat oxidation and inhibits the differentiation of pre-fat cells into mature fat cells

•       Visceral fat regulation — low testosterone is specifically associated with increased visceral (abdominal) fat accumulation

•       Insulin sensitivity — testosterone improves the body's response to insulin, which is critical for blood sugar regulation and fat burning

•       Metabolic rate — because testosterone supports muscle mass, and muscle burns more calories than fat at rest, optimal testosterone supports a higher baseline metabolic rate

The Vicious Cycle of Low T and Weight Gain

One of the most challenging aspects of low testosterone and weight gain is that each makes the other worse:

1.    Low testosterone reduces muscle mass and lowers metabolic rate

2.    With a slower metabolism, the body stores more fat — particularly visceral fat

3.    Visceral fat contains high levels of aromatase — an enzyme that converts testosterone to estrogen

4.    Elevated estrogen from aromatization further suppresses testosterone production

5.    Lower testosterone means less muscle, more fat storage, and the cycle continues

Breaking this cycle requires addressing both components — the hormonal imbalance and the excess weight — simultaneously. Treating just one often produces limited results because the other continues to work against you.

Weight loss in obese men can increase testosterone levels by 50–100 ng/dL per 10 kg lost. Conversely, testosterone therapy in men with low T significantly reduces fat mass while increasing lean muscle — creating a powerful synergistic effect when both are addressed together.

When to Consider GLP-1 Therapy Alongside TRT

For men with significant weight to lose — particularly those with a BMI over 27 with metabolic comorbidities — combining TRT with a medically supervised GLP-1 weight loss program can produce results that neither approach alone can achieve.

GLP-1 medications like semaglutide and tirzepatide correct the metabolic signaling that drives hunger and fat storage, allowing men to lose weight effectively even when conventional diet and exercise haven't been sufficient. When combined with TRT, the benefits compound:

•       TRT preserves and builds lean muscle mass while GLP-1 therapy drives fat loss — resulting in body composition changes rather than just weight loss

•       Improving testosterone levels reduces the aromatase activity driving estrogen production, supporting more favorable hormonal balance

•       Weight loss from GLP-1 therapy can itself improve testosterone levels, creating a positive feedback loop

•       Better energy and motivation from TRT supports adherence to the nutrition and exercise components of the weight loss program

Protecting Muscle During Weight Loss

The biggest risk with any significant weight loss program — including GLP-1 therapy — is muscle loss alongside fat loss. For men, this is especially important because muscle mass is directly tied to testosterone response, metabolic rate, and long-term health.

At JS Wellness, we prioritize muscle preservation throughout our weight loss programs:

•       Protein target of 100+ grams per day — protein is the single most important dietary factor in preserving muscle during caloric deficit

•       Resistance training guidance — strength training signals the body to maintain muscle even during weight loss

•       Body composition monitoring — we track muscle and fat separately, not just total weight

•       TRT dose optimization — ensuring testosterone levels are optimal throughout the weight loss process supports muscle retention

What Our Combined Approach Looks Like

At JS wellness, we're one of the few clinics in the Lawrence and Kansas City area that offers both TRT and medically supervised GLP-1 weight loss under the same roof. This means we can address the hormonal and metabolic dimensions of your situation simultaneously — with a single provider who understands how they interact.

A typical combined program includes:

•       Comprehensive lab work — testosterone, estradiol, metabolic panel, insulin sensitivity markers, and more

•       TRT initiation — injectable testosterone or hormone pellets, dosed to your individual labs

•       GLP-1 prescription — semaglutide or tirzepatide, started at a conservative dose and titrated based on your response

•       Monthly check-ins with Julie — reviewing labs, adjusting doses, tracking body composition

•       Nutrition and exercise guidance — protein targets, strength training recommendations, and practical dietary advice

•       Ongoing monitoring — to ensure your hormonal and metabolic profile is trending in the right direction throughout

Ready to Break the Cycle?

If you've been struggling with weight, body composition, or the symptoms of low testosterone — or if you suspect both are playing a role — the most productive next step is a comprehensive lab evaluation.

At JS wellness in Eudora, KS, we'll tell you honestly what your labs show and what a plan built around your biology, your goals, and your life actually looks like.
