The Protein Priority: How to Eat for Weight Loss That Lasts

The dream of weight loss is simple: lose the fat, keep the muscle. The reality is more complicated — because under the wrong conditions, significant calorie reduction from any source (including GLP-1 medications) can cause the body to break down muscle as well as fat.


At JS wellness in Eudora, KS, protecting our patients' muscle mass is one of the central goals of our medical weight loss programs. Not just because it affects how you look — but because muscle is the foundation of your metabolism, your long-term health, and your ability to maintain your weight loss results for the years ahead.


Here's exactly how we approach it — and what you can do to make sure the weight you lose is the weight you actually want to lose.


Why Muscle Loss Happens During Weight Loss

When your body is in a caloric deficit — consuming fewer calories than it burns — it needs to find fuel from somewhere. Ideally, that fuel comes primarily from stored fat. But the body is opportunistic: it will also break down muscle tissue for fuel, particularly when:

•       Protein intake is inadequate — without enough dietary protein, the body cannibalizes muscle to meet its amino acid needs

•       Exercise doesn't signal muscle preservation — without a stimulus to maintain muscle (primarily resistance training), the body treats it as expendable tissue

•       Weight loss is too rapid — very fast weight loss is more likely to include significant muscle loss

•       Testosterone or other anabolic hormones are low — hormonal deficiency reduces the body's ability to build and preserve muscle


GLP-1 medications are generally associated with a more favorable muscle-to-fat loss ratio than many other weight loss approaches — particularly when protein intake is adequate. A 2025 genetic study confirmed that GLP-1 receptor agonists achieve weight loss primarily by reducing fat mass more than muscle. But this advantage is conditional: it requires sufficient protein and exercise to fully realize.


The difference between losing 30 pounds of fat while preserving muscle and losing 20 pounds of fat plus 10 pounds of muscle is enormous — both for how you look and feel, and for your long-term metabolic health. Protecting your muscle is not optional. It's the whole game.


The Three Pillars of Muscle Preservation

1. Protein — 100 Grams Per Day, Minimum

Dietary protein provides the amino acids your body needs to repair, maintain, and build muscle tissue. During caloric restriction, adequate protein intake signals the body to preferentially burn fat rather than muscle — a process called protein-sparing.


Our minimum recommendation: 100 grams of protein per day for all GLP-1 patients. For patients who are larger, more active, or have higher muscle mass, we may recommend up to 1.2–1.6 grams per kilogram of ideal body weight. Protein should be the first priority at every meal — eaten before carbohydrates or fats to ensure it's consumed even when appetite is suppressed.


2. Resistance Training — The Signal Your Body Needs

Protein provides the raw material for muscle preservation. Resistance training provides the signal. When you challenge your muscles with weight, resistance bands, or bodyweight exercises, you trigger a cascade of hormonal and cellular signals that tell your body: this tissue is being used, it is necessary, preserve it.


Without that signal, even adequate protein intake may not be enough to fully prevent muscle loss during significant caloric restriction.


Our recommendations for resistance training during GLP-1 therapy:

•       Minimum 2–3 resistance training sessions per week — more is better, but consistency matters more than volume

•       Focus on compound movements that engage multiple muscle groups: squats, deadlifts, rows, presses, and lunges

•       Progressive overload — gradually increasing the challenge over time (more weight, more reps, or less rest) is what drives continued muscle adaptation

•       Don't skip upper body — many patients focus only on cardio or lower body; include pushing and pulling movements for the chest, back, and shoulders

•       Bodyweight workouts count — if gym access is limited, push-ups, split squats, rows with bands, and planks provide a meaningful muscle preservation signal


3. Monitoring Body Composition — Not Just Weight

The scale tells you how much you weigh. It doesn't tell you what you're losing. Two patients can lose the same amount of weight with dramatically different outcomes: one loses primarily fat while preserving muscle, the other loses a significant proportion of muscle alongside fat.

The Hormone Connection

Muscle preservation is not just about protein and exercise. Hormones play a critical role — and this is one of the reasons why our integrated approach at JS wellness, which addresses both weight loss and hormone optimization, consistently produces better body composition outcomes than weight loss programs that treat the two in isolation.

Testosterone is an anabolic hormone — it directly stimulates muscle protein synthesis and inhibits muscle breakdown. In both men and women, low testosterone makes muscle preservation during weight loss significantly more difficult. If you're losing weight on a GLP-1 program and finding it very hard to maintain muscle despite adequate protein and resistance training, it's worth evaluating your testosterone levels as part of the picture.


Similarly, thyroid hormones regulate overall metabolic rate and protein metabolism. Low thyroid function can impair muscle protein synthesis and contribute to the fatigue that makes consistent exercise difficult.


Signs You May Be Losing Muscle

Watch for these signals during your weight loss program — they may indicate that muscle preservation strategies need to be strengthened:

•       Feeling weaker at the gym or in daily activities despite continued weight loss

•       Significant fatigue that goes beyond normal exercise recovery

•       Rapid weight loss without corresponding improvement in how you look (weight loss without visible fat reduction)

•       Significant decrease in exercise performance — less strength, less endurance

Sleep: The majority of muscle repair and growth happens during deep sleep, when growth hormone secretion peaks. Chronic sleep deprivation significantly impairs muscle recovery and protein synthesis. Aim for 7–9 hours of quality sleep per night — and if sleep is disrupted (a common symptom of hormonal imbalance), address it as part of your overall plan.


Stress: Elevated cortisol — the body's primary stress hormone — directly promotes muscle breakdown and fat storage, particularly around the midsection. Chronic stress actively works against muscle preservation and weight loss simultaneously. Stress management practices (exercise, adequate sleep, social connection, mindfulness) are not optional add-ons — they're part of the metabolic equation.


What a Successful Outcome Looks Like

Patients who follow these principles consistently — adequate protein, regular resistance training, hormonal optimization when needed, quality sleep — tend to see body composition outcomes that look and feel dramatically different from patients who focus only on the scale.


They lose weight, but they maintain or even improve their muscle tone. Their clothes fit differently in ways that reflect actual fat reduction. Their energy improves rather than declining. And when they reach their goal weight, they have a body that functions well, maintains its results, and continues to feel strong.
